COVID Vaccination Register for Students

As the current situation stands any student aged 12 years and 3 months or over may need to be double vaccinated to access activities and services outside of the school.

Providers and event organisers (such as camps, trips, sports event organisers) not employed by the Ministry of Education can require that any young person who is 12 years, 3 months and over to be double vaccinated so that they can access their events or services.

WAINUI CAMP 2022 - VERY IMPORTANT INFORMATION:

For our Year 8 students attending camp Wainui in 2022 the provider is requiring all students who are 12 years & 3 months or older, when they go to camp to be double covid19 vaccinated. We, the school need to receive proof of their vaccination - their vaccination passport or ministry of health letter otherwise they will not be able to attend.

For proof of your child's vaccination please submit a copy of their vaccination confirmation from the Ministry of Health or their 'My Vaccine' Pass. Both can be obtained immediately after they are double vaccinated. A screen shot or scan is sufficient in the current situation. The links are as follows:

The information regarding your child's COVID-19 vaccination status will be held and managed by the Casebrook Intermediate School in accordance with the Privacy Act 2020 and Health Information Privacy Code 2020.
